What is an M&M Review?

An M&M (aka Morbidity and Mortality) Review is a century-old practice that originated in the medical field. The purpose of an M&M Review is to learn from difficulties in prior cases as a way of improving care to current and future patients. The goal is to arrive at this learning in a psychologically safe, non-blaming atmosphere. Rather than focus on regret and self-blame following an adverse event, the aim is to examine systems and processes that can benefit patient care moving forward.

Another way that this process can be helpful is to identify and mitigate vulnerabilities in your practice in order to reduce the chances of an adverse event in the future. Or, perhaps you want to use the time simply to learn more about risk management.

Understanding Adverse Events

What is an adverse event? If you’ve experienced one, you probably know what this is. The adverse event might not be as well-defined as events in the medical field, where the wrong leg may have been amputated. Perhaps the most common form of adverse events in mental health practices involve a client or family member who is disgruntled or unhappy with you. Other expressions of adverse events may involve a threat, whether direct or veiled. 1

If you have experienced an adverse event in your practice and wish to schedule an M&M Review with us, we will invite you to complete a form that allows you to describe the adverse event(s) – without identifying client information – prior to our meeting. We will also gather details about your practice such as your consent forms, intake forms, and intake processes. 



Benefits of an M&M Review

Even if you have been fortunate enough not to have experienced an adverse event in your practice, you may want to process client experiences that concern you, even if they do not rise to the level of “adverse” – which is another way you can benefit from the M&M process. The consult may help to identify and mitigate current vulnerabilities to reduce the risk of an adverse event down the road. The review process can also be helpful if you find yourself struggling to maintain boundaries in your practice.

Please note that if you are in the midst of a formal complaint, it is best to work with an attorney in your state for strategies about how to respond. Our role as M&M consultants would involve reviewing the systems and processes in your practice more generally in terms of how updates may reduce vulnerabilities moving forward.
